Do you find it frustratingly difficult to fall asleep at night? Wake frequently during the night? Weary-eyed at 4AM, without being able to fall back asleep?
The many variations on the theme of insomnia are a major health concern, to the point where the CDC has declared it a public health epidemic. Americans are notoriously sleep-deprived, and the stats in this country are staggering:
- Approx. 30% of adults have symptoms of insomnia
- Approx. 10% of the population may suffer from long-standing insomnia
- Insomnia costs the workforce $63.2 billion a year in lost productivity
- Insomnia is a major contributing factor to deaths caused by motor vehicle crashes
- Roughly 27% of working women suffer from insomnia, compared to 20% of working men
Insomnia is defined as: “Difficulty initiating or maintaining sleep, or both, despite adequate opportunity and time to sleep, leading to impaired daytime functioning.” Sound like you fit the bill?
In my practice, I see lack of good sleep likely undermining or being a contributing factor in many conditions, including chronic fatigue, anxiety, depression, and IBS. In fact, studies have shown that ongoing sleep deficiency is linked to an increased risk of heart disease, kidney disease, high blood pressure, diabetes, obesity, and stroke. Insomnia can also increase the risk of car crashes, work-related accidents, and cancer.
How Much Sleep is Enough?
Here are the recommended number of hours of sleep per age group:
- 0-1 year: 12-16 hours/day (including naps)
- 1-3 yrs: 11-14 hours/day (including naps)
- 3-5 yrs: 10-13 hours/day (including naps)
- 6-12 yrs: 9-12 hours/day
- 13-18 yrs: 8-10 hours/day
- Adults: 7-8 hours/day
Bottom Line: The right quantity and quality of sleep is worth it!
The Benefits of Restorative Sleep
- Healthy growth and development in kids
- Proper healing and repair of your heart and blood vessels
- Improved insulin response – this is the hormone that controls your blood sugar level
- Improved immune function, enabling your body to fight infections with more oomph and keep inflammation in check
- Better ability to think, learn, work, and be actively and positively engaged with your environment.
The Stages of Sleep
Sleep occurs in cycles, alternating between various stages throughout the night. While the exact time spent in each stage varies, here’s a general breakdown over a typical 7-9 hour sleep cycle:
- NREM Stage 1 (Light Sleep): This stage lasts 5-10% of total sleep. It’s the brief transition from wakefulness to sleep, where your body begins to relax, and brain activity slows. You’re easily awakened in this phase.
- NREM Stage 2 (Deeper Light Sleep): Accounting for 45-55% of total sleep, this is the longest sleep phase. Heart rate and body temperature decrease, and the body prepares for deeper rest.
- NREM Stage 3 (Deep Sleep): This phase makes up 13-23% of total sleep. Also called slow-wave sleep, this stage is the most restorative, supporting physical recovery, tissue repair, and immune function. You experience the most deep sleep in the first half of the night.
- REM Sleep: Making up 20-25% of total sleep, REM sleep is vital for brain function, memory consolidation, and emotional processing. REM sleep periods lengthen in the second half of the night, with the longest REM cycles occurring closer to morning. This is also the dream stage.
Sleep cycles last about 90 minutes, and as the night progresses, REM periods grow longer, while deep sleep phases shorten. The balance of sleep stages—known as sleep architecture—plays a crucial role in cognitive function. Insufficient deep sleep can lead to memory issues, while a lack of REM sleep affects emotional regulation.
What Happens in the Brain During Sleep?
Sleep is much more than just “resting.” It’s a highly active period where both the brain and body perform essential functions:
- Memory consolidation: Sleep helps organize and store memories. REM sleep processes emotional memories, while non-REM sleep focuses on factual memories.
- Waste removal: The glymphatic system, aka. the brain’s lymphatic system, clears out toxins, such as beta-amyloid proteins, which are linked to Alzheimer’s disease. This occurs primarily during deep sleep.
- Emotional regulation: Adequate sleep stabilizes mood and improves emotional resilience, while poor sleep can increase anxiety and stress.
- Hormone regulation: Sleep controls the production of key hormones like cortisol (stress), ghrelin (hunger), and leptin (satiety), which impact weight and stress management.
Insights on Sleep Disorders
Many sleep disorders stem from neurological or psychiatric causes. Psychiatric-based sleep disorders, in particular, can be heavily influenced by stress and anxiety. Here are some of the main sleep disorders and their common causes:
- Insomnia: Difficulty falling asleep, staying asleep, or waking up too early. Common causes include stress, poor sleep habits, mental health issues (like anxiety or depression), and medical conditions such as chronic pain or respiratory problems.
- Sleep Apnea: Repeated interruptions in breathing during sleep, leading to poor rest. Obstructive sleep apnea (OSA) occurs when airways are blocked, while central sleep apnea (CSA) results from the brain failing to signal muscles to breathe.
- Restless Leg Syndrome (RLS): An uncontrollable urge to move the legs, often with discomfort, especially at night. Causes include neurological imbalances, iron deficiency, pregnancy, or genetic factors.
- Narcolepsy: A chronic condition characterized by excessive daytime sleepiness and sudden sleep attacks. It’s often linked to a deficiency in hypocretin (a neurotransmitter regulating wakefulness), genetics, or autoimmune factors.
- Circadian Rhythm Disorders: Misalignment of the body’s internal clock, affecting sleep timing. Causes include shift work, jet lag, and excessive exposure to light at night.
- Parasomnias: Abnormal behaviors during sleep, such as sleepwalking or night terrors. These may be caused by genetics, sleep deprivation, or stress.
- REM Sleep Behavior Disorder (RBD): Acting out vivid dreams during REM sleep, sometimes violently. This is associated with neurodegenerative diseases like Parkinson’s or triggered by certain medications or withdrawal from sedatives.
- Hypersomnia: Excessive daytime sleepiness despite adequate nighttime sleep. Causes include sleep apnea, medical conditions like depression or hypothyroidism, or a genetic predisposition.
The Problem with Sleep Medications
Prescription sleep medications, like Ambien (zolpidem) and Trazodone, are often used to address insomnia, but they come with significant downsides, especially when used long-term.
- Dependency and tolerance: Regular use of these medications can lead to tolerance, where you need higher doses to achieve the same effect. This increases the risk of dependency, making it harder to sleep without them.
- Disruption of sleep architecture: While these drugs can help you fall asleep faster, they disrupt the natural stages of sleep. For example, Ambien can decrease deep sleep and REM sleep—the restorative stages crucial for memory consolidation and emotional processing. This results in less restorative sleep, even if you’re unconscious for a full night.
- Side effects and next-day drowsiness: Ambien is associated with side effects like sleepwalking, memory lapses, and morning drowsiness. Trazodone, originally an antidepressant, is commonly used off-label for sleep but can leave you feeling groggy with side effects like dry mouth, dizziness, and low blood pressure the next day.
- Cognitive and physical health risks: Prolonged use of these medications is linked to cognitive impairment, memory issues, and balance problems, which increase the risk of falls, especially in older adults.
- Rebound insomnia: When discontinuing these drugs, users often experience rebound insomnia, a condition where sleep problems return worse than before. This can trap individuals in a cycle of medication dependence.
While sleep medications can provide short-term relief, they are not a sustainable long-term solution. Better sleep hygiene, relaxation techniques, and cognitive behavioral therapy (CBT) are safer, more effective ways to improve sleep quality without the risks associated with medications.
General Sleep Tips for Optimization
Here are some tried-and-true methods to help encourage great sleep. If you still need help rebooting your sleep after implementing these, discuss this with your physician. It is imperative, for your longevity’s sake, that you get solid sleep!
- Get morning sunlight: Exposure to sunlight in the morning halts the release of melatonin in your brain, helping you wake up and regulating your circadian rhythm. To take advantage of this natural cue, step outside or sit by a window for direct sunlight within the first 30 minutes after waking. This simple practice can significantly enhance your alertness and overall well-being throughout the day.
- Get morning exercise: Incorporating exercise into your daily routine can strengthen your circadian rhythm, boost daytime alertness, and help you feel sleepy when it’s time for bed. Morning workouts are particularly beneficial, as they align with your body’s natural rhythm. However, it’s best to avoid intense exercise right before bedtime, as it can elevate your body temperature and disrupt sleep. Aim to complete your workout in the morning to maximize the benefits for both your physical health and sleep quality.
- Focus on diet: Light, early dinners support optimal sleep. Avoid large meals, sugary desserts, and spicy foods late at night, as they can cause discomfort, glucose and cortisol spikes, and indigestion.
- Limit caffeine intake: To improve your sleep, avoid caffeine after 2 PM. The half-life of caffeine is approximately 6 to 8 hours, meaning it can remain in your system long after consumption. For instance, if you drink an 8 oz cup of coffee containing about 100 mg of caffeine at 4 PM, you’ll still have around 50 mg in your body by 10 PM, which can disrupt your sleep schedule. To promote better sleep quality, aim to stop caffeine consumption well before bedtime.
- Avoid alcohol before bedtime: Be mindful of your alcohol consumption, especially within three hours of bedtime. While it may help you fall asleep initially, alcohol can disrupt REM sleep later in the night, impacting sleep quality. Enjoying an occasional drink is usually fine, but indulging too close to bedtime can lead to issues. If your body is still digesting alcohol when you lie down, it can hinder the restorative benefits of your early sleep. Ideally, aim to finish your last drink at least three hours before lights out to minimize any negative effects on your sleep quality.
- Wind down with a bedtime routine: Incorporate calming pre-sleep rituals such as reading, meditation, or gentle stretching. Avoid stimulating activities before bed.
- Create a consistent sleep schedule that allows for 7.5-8.5 hours of sleep: Aim for 7.5 to 8.5 hours of sleep each night by going to bed and waking up at the same time daily, even on weekends. This consistency is essential for regulating your body’s internal clock and maintaining a healthy circadian rhythm . When you rise each morning, exposure to light helps reset your circadian rhythm, signaling your brain to prepare for the day ahead . Sticking to a regular sleep schedule, seven days a week, lays the foundation for better sleep quality and overall well-being. By creating this routine, your brain learns when to expect sleep and wakefulness, leading to more restorative rest.
- Optimize your sleep environment:
- Your bed, pillow, sheets, and blanket should be as clean, natural, and organic as possible. This reduces exposure to toxic chemicals, including formaldehyde, benzene, and naphthalene.
- Keep your wifi off at night. Here’s why. This is particularly important for individuals who have a known sensitivity to EMFs. But, all of our brains would surely benefit from reduced exposure, especially at night, when our bodies are in repair mode.
- Keep the bedroom cool (between 65 and 75 degrees F), dark, and quiet, and open your window a bit, even if it’s just a sliver, so that the well-oxygenated air from outside can circulate through the bedroom.
- Limit exposure to blue light (from screens) 2 hours before bed.
- Use blackout curtains, earplugs, or white noise machines as needed.
If you still need some extra supplemental support:
- My go-to supplement is NapCaps, which contains herbal extracts of valerian, passionflower, and lemon balm, as well as L-Theanine and 5-HTP. It works wonders for many of my patients (and me)! Magnesium and melatonin can also work well, taken before bedtime (discuss with your doc).
- 1-2 teaspoons of a gentle lemon balm & chamomile tincture or a cup of tea from these herbs can also be lovely, taken 15 minutes before bed.
It’s ALWAYS important to focus on any underlying factor(s) or conditions egging on that nighttime wakefulness. Adrenal hormone and melatonin level testing can help to determine abnormalities in your circadian rhythm cycling (ie. you should have the “wake” hormones be high during the day and the “sleep” hormones high during the night, but sometimes these levels are turned around or are otherwise abnormal). I use this type of testing often in my office, and have found it to be a foundational part of the assessment process for insomnia.
Why Not Try A Sleep Challenge?
A sleep challenge is a great way to take a month to prioritize your sleep and see how this ripples out into the quality of your days. Here’s how:
Week 1: Baseline & Goal Setting
Track your current sleep habits using a sleep diary or sleep tracker (Oura Ring, Apple Watch, etc.). Set specific sleep goals for 1 week, 2 weeks, and 1 month.
Week 2: Sleep Hygiene Optimization
Establish a consistent sleep schedule, cut out screens before bed, and create an optimal sleep environment. The simplest way to encourage longer sleep is by going to bed earlier- start by moving your bedtime earlier by 15 minutes.
Week 3: Advanced Sleep Strategies
Incorporate relaxation techniques such as meditation or deep breathing. If needed, try supplementing with magnesium, melatonin or an herbal sleep aid.
Week 4: Celebrate Progress & Plan for the Future
Review your progress by comparing your current sleep habits to your baseline. Continue practicing good sleep hygiene and tracking your sleep data for long-term improvement.
